Q1 What do I need to bring to an appointment when seeing an Endocrinologist?

  • In order to receive a Medicare rebate, you will need to bring a valid referral. A referral from your General Practitioner (GP) is typically valid for 12 months but your GP can also make it ‘indefinite’. A referral from another specialist is only valid for 3 months
  • The name and contact details of other health professionals you currently see
  • Medicare card, DVA card, and/or Pension card
  • Results of any recent investigations, including: blood tests, urine analysis, x-rays, and scans

 

 

Q2 What do I need to bring to an appointment when seeing the Diabetes Educator?

  • If you are being referred to the Diabetes Educator and have Type 1 or Type 2 diabetes then you will need to bring a valid “Chronic Disease Management Plan” from your GP in order to receive a Medicare rebate. Please also check with your private health insurance as some policies include cover for allied health consultations
  • The name and contact details of other health professionals you currently see
  • Medicare care, DVA card, and/or Pension card
  • Results of any recent investigations, including: blood tests, urine analysis, x-rays, and scans

 

 

Q3 How do I make an appointment to see a health professional?

You can use one of the following options in order to make an appointment:

  • Fax your referral letter to (03) 9326 8922 , ensuring it has your contact number

 

 

Q4 Will I receive confirmation of my appointment?

Once you have contacted Victorian Endocrine Clinic and have scheduled an appointment, you will be posted a confirmation letter. The letter will detail the date and time of your appointment, consultation fees and practice location. In addition, prior to all appointments a confirmation text will be sent to your mobile phone.

 

 

Q5 How do I pay for my appointment?

Payment is required in full, on the day of consultation. Payment can be made by cash, credit card, eftpos or cheque.

If you have already registered your banking details with Medicare then we can electronically lodge your rebate claim on the day of your consultation.

 

 

Q6 How can I reschedule or cancel an appointment?

If you wish to reschedule an appointment please contact our reception staff directly on (03) 9326 8622. Cancellations and rescheduling requires at least 24 hours’ notice.

 

Q7 What happens if I miss my appointment?

We expect that if you cannot attend an appointment you ring the rooms at least 24 hours prior to the appointment to cancel it as this allows the appointment to be available for someone else.  Non attendance may result in you being billed a non-rebatable standard consultation fee for a missed appointment.
